To print the form which takes two pieces of paper do one of the following:
1) go to your tool bar (usually at the top of your screen) and press the 'Print' button or
2 ) Go to 'FILE' and request 'Print' from the drop down menu or
3) If you are using a PC hold down the following keys on your keyboard at the same time: 'Ctrl' and the letter 'P'
If you are using an Apple Macintosh hold down 'Apple' (it has a little apple logo) and the letter 'P'
